Reg. 381/12 (FORMS) 1. Ontario Regulation 381/12 is amended by adding the following section: Minor improvement project 5.1 A notice of request for a minor improvement project under subsection 78 (5) of the Act shall be in Form 9. 2. The Table to the Regulation is amended by adding the following item:   9 Notice of Request for Drain Minor Improvement / Avis de demande d’amélioration d’importance mineure d’installations de drainage 2021/06   Commencement 3.
