2   Persons to whom these contracts apply:

(a) employees whose eligibility has been established by a collective agreement, employer directive or order of the Lieutenant Governor in Council;

(b) eligible bargaining unit employees of British Columbia Ferry Services Inc.;

3   (1) The following rates of contribution and coverage apply to those employees or persons described in section 2:

Conditions

4   Conditions affecting those employees covered by these plans:

(a) employees granted leave of absence without pay for a period of a full calendar month or more may continue coverage for a maximum duration of 24 months, provided that the employee pays the full cost during the period of absence;

(a.1) despite paragraph (a), an employee is not required to pay the full cost if the leave of absence is for the purpose of working for or volunteering with CUSO;

(b) when a regular employee becomes an auxiliary employee, benefits under these plans shall cease at the end of the month in which the change is made, except where continuation of benefits is provided for in an applicable collective agreement, employer directive, or order of the Lieutenant Governor in Council;

(c) when a covered employee transfers without a break in service from one group under these plans to another group also under these plans, coverage shall continue uninterrupted;

(d) when a covered employee has a break in service of less than one full calendar month and returns to service to either the same group or another group covered under these plans, coverage shall continue uninterrupted;

(e) the effective date of coverage under these plans for an employee who is eligible for benefits, but who has not submitted a completed application form, is the first day of the month following the month in which the completed application form is received by the employer;

(f) for persons referred to in section 2 (s), coverage is limited to 6 months following the date the privatized venture begins operating in the private sector;

(g) regular or auxiliary employees who are on temporary layoff for a period of a full calendar month or more may continue coverage for the period specified in the applicable collective agreement, employer directive, or order of the Lieutenant Governor in council, provided that the employee pays the full cost during the period of layoff.
